HELMET AUCTION

COLLIN ZWICKERT

Due to injuries sustained at birth Collin lost heart rate and breathing.  It took 22 minutes to restore both.  
Collin was given 1 hour to live.  
As a result, Collin suffered a severe brain injury leaving him with many medical, physical and mental
obstacles to overcome.  

Prior to delivery Collin was 100% healthy.
Collin's Diagnosis’s:
Umbrella diagnosis is Cerebral Palsy.  
Underlying issues:
•        Hypoxic Ischemic Encephalopathy
•        Spastic Quadriplegic Cerebral Palsy
•        Gastroesophageal Reflux Disease
•        Cortical Vision Impairment
•        Central Hearing Loss
•        Dysphasia (can’t swallow)
•        Hip Dysplasia
•        Possible Chronic Pneumonia due to
Saliva Aspiration
 Daily Medications/Treatments:
•        Speech Therapy (2x/wk)
•        Occupational Therapy (1x/wk)
•        Physical Therapy (2x/wk)
•        Vision Therapy (2x/wk)
Medications:
•        Robinul (to dry up saliva)
•        Zantac (reflux)
•        Albuterol (breathing)
•        Atrovent (breathing)
•        Pulmicort (breathing)
•        Tobimyicin
•        Tylenol/Motrin (discomfort)
•        Diazepam (for spasms)
•        Baclophen (for spasms)
Not Covered:
•        Cranial Sacral Therapy  (2x/month)
•        Vital Stim ( 2x/week)

•        Hip Surgery with dual hip & leg casts
for 4 months
•        Hip brace for 4-6 months

Future Procedures:
•        Bi-Lateral Hip Surgery - January 2009
•        Botox injections into saliva glands to
reduce saliva production
•        Botox injections into his extremities
to reduce spasticity
Bidding will close on Friday January 16, 2009 at noon.  Winning bids to be announced by United Cerebral
Palsy of Delaware staff @ US Lacrosse convention In Baltimore, MD that same evening.

Winning bidders will be notified and a certified check or money order for the winning bid will be sent to UCP
of Delaware in Collin Zwickert's name (on behalf of the winning bidder) and the helmet will be shipped to
you upon receipt of the funds.
